On May 22, 2006, a Notice of Violation was issued for a Severity Level III violation involving the failure to secure from unauthorized removal or limit access to a portable gauge containing licensed material, while the gauge was in an unrestricted area and not in storage; nor did the licensee control and maintain constant surveillance of this licensed material. While the gauge was unattended, it was run over by a front end loader and destroyed.
